A Florida Home Care and Home Health Agency that provides both NON-MEDICAL and SKILLED Home Care is seeking a highly motivated and energetic Case Manager Liaison to conduct outreach and build relationships with managed care plans and other payors throughout the State of Florida. The Case Manager Liaison is responsible for conducting consistent and effective outreach to managed care case managers, as well as case managers at other payors, to communicate the Company’s care offerings, obtain new patient referrals and coordinate activity between the case managers and the Company’s offices to ensure care can be started.

  • Represent the Company in a professional and courteous manner in all interactions with clients/patients, referral sources, accounts, and customers.
  • Cultivate business relationships with potential referral sources and clients/patients; referral sources for this position would specifically be case managers (and other smiliarly positioned employees) at insurance plans or other payors who manage the home care of their patients and refer patients to Company for that care.
  • Communicate effectively with Operations, Intake and Customer Service (which would include for example, coordinating across departments to ensure all patient and insurance documentation is obtained timely for start of care).
  • Search out, identify and establish communication with all potential case managers at each of the different plans/payors.
  • Maintain a current listing of all contact with contact information and activity and submit that report to your supervisor weekly.
  • Develop and retain existing business relationships with the case managers.
  • Effectively communicate the Company’s care offerings, capabilities and value proposition to the case managers.
  • Achieve goals set by management for minimum number of new referrals and patient admissions per month.
  • Represent the Company at specified Case Management meetings and industry events in a manner that will enhance the Company’s standing with the managed care plans.
  • Ensure ongoing compliance with all laws and regulations; ensure that assigned area of responsibility meets or exceeds accreditation standards; and implement ‘’best practices’’ in all activities.  
  • Perform other duties as assigned by your supervisor.
  • Ability to organize and plan outreach/marketing activities within your assigned area(s).
  • Ability to research and identify new account opportunities and prospects.
  • Ability to work both independently in building your customer relationships, as well as with a team.
  • Excellent communication and interpersonal skills.
  • Preferably a minimum of two (2) years of experience as a case manager with an insurance plan, health system, or similar organization.
  • Excellent computer skills that include Excel, Word, efficient use of the internet and e-mail, and ability to utilize a CRM software if applicable.
