The Patient Care Coordinator is expected to perform a variety of duties that include the coordination of scheduling services for Clients through Caregivers in a high paced environment. The Coordinator is responsible for working with Clients and Caregivers in order to provide the highest quality service to clients with emphasis on creating extraordinary relationships. Most of the job responsibility is performed by phone and computer.- In Office position, not remote
- Bi-Lingual (Spanish) preferred

Responsibilities
- Receives referrals from Private, Insurance and Contracting Agencies.
- Provides appropriate field staff for all cases.
- Orientation of all employees to their cases.
- Schedules caregivers to the appropriate patients.
- Assignment of cases to RN Field Supervisor for initial assessment and case management.
- Maintains all necessary communication and documentation.
- Maintains and update employee records in compliance with the State Department of Health and accrediting body standards.
- Maintains accurate client and employee information in the computer.
- Reports all client incidents / accidents to the nursing supervisor for follow up.
- Handles problems that may arise concerning clients, families, field staff, contracting agencies and any other persons or organizations with whom the coordinator interacts.
- Be available to be on call in case of emergencies or unavailability of on-call coordinator.
- Maintains compliance with all agency policies and procedures.
- Performs other duties as mutually agreed.
Skills & Qualifications
Position requirements:- Previous scheduling Home Health and/or Call Center experience preferred
- Good organizational and communication skills
- Ability to multi-task and handle a high call volume in a fast paced environment
- Must be open to work evenings and week-ends on a rotating basis
- Bi-Lingual (Spanish) preferred
- Visual / hearing ability sufficient to comprehend written / verbal communication.
- Computer and data entry skills.
- Ability to perform repetitive tasks.
- Ability to effectively deal with stress.
- Ability to verbalize so that the average person can comprehend.
- Able to sit for long periods of time.
- Carry out a wide variety of tasks with frequent interaction with many people.
